On March 23, 2023, UI Community HomeCare discovered encrypted files on certain computer systems. UI Community HomeCare immediately launched an internal investigation to determine the nature and scope of the activity. The investigation determined there was unauthorized access to UI Community HomeCare’s servers beginning on or about March 23, 2023 and its systems and identified the event in a timely manner. UI Community HomeCare has determined that the impacted files contained personal information related to patients. At this time, UI Community HomeCare sees no evidence of misuse of any information related to this incident. UI Community HomeCare is performing a comprehensive security review of its systems and will take steps to prevent a similar event occurring in the future.

All impacted individuals with a valid postal address have been notified by mail. UI Community HomeCare uses a separate electronic health record system from University of Iowa Health Care. UI Health Care patient information was not impacted by this event.

The scope of information potentially involved includes name, date of birth, address, phone number, medical record number, referring physician, dates of service, health insurance information, billing and claims information, medical history information, and diagnosis/treatment information. UI Community HomeCare advises individuals who have received UI Community HomeCare services to take steps to help prevent and detect misuse of information. The following actions are recommended to help prevent misuse of your information:

• Closely monitor any "Explanation of Benefits" or "EOB" sent by your insurance company or other entity that pays your medical bills and contact UI Community HomeCare if there are claims for services you did not receive.
• Report any suspicious activity to your insurance company or health care provider.

UI Community HomeCare understands the serious nature of any potential breach – no matter how limited – so it has conducted a thorough investigation, identified, and mitigated the risks, and strengthened its security oversight efforts to minimize the likelihood of a similar occurrence. UI Community HomeCare values patient privacy and deeply regrets any inconvenience this may have caused patients and their families.
